Dove Physical Therapy, LLC is a fee-for-service outpatient physical therapy clinic.  Dove Physical Therapy, LLC is “in-network” with Blue Cross Blue Shield of Alabama, TRICARE, Aetna, and Veterans Affairs (VA) Community Care Network (CCN).  Patient

co-payment or co-insurance is due at the time of service, and the office will submit your insurance claim as a courtesy to you.  Self-payment options are also available for patients who do not have insurance coverage.  Since we are out-of-network and not affiliated with all insurance providers, the full fee or payment is due at the time of service.  Patients will receive a Patient Receipt for physical therapy services rendered in order to submit their claims to insurance companies for direct reimbursement or reimbursement from a flex spending account.  If physical therapy is a medical necessity, why not receive private, one-on-one care?  Please note that we have no relationship with Medicare or Medicaid insurance providers. Dove Physical Therapy, LLC's mission is to always provide private, one-on-one hands-on High Quality Physical Therapy Care (HQPTC). Choosing Dove Physical Therapy, LLC as your healthcare provider for physical therapy treatment will ensure that each therapy session will be private, one-on-one, and hands-on. This means you will not share a treatment session with another patient at any time.

The Plan of Care (POC) may include 60 minutes treatment sessions and/or mini treatment sessions. Mini treatment sessions are based on the duration of the session.  During the treatment session(s),  specific, isolated needs of the patients' condition are pinpointed. The fees of these highly specialized mini treatment sessions varies.  Other mini treatment sessions can be added to the normal 60 minutes sessions.
