Dove Physical Therapy, LLC is glad that you are choosing us as your preferred healthcare provider and scheduling an appointment with us. But before you schedule an appointment, please discuss your needs and concerns with your physician first; and then, visit the Patients, Patient Forms, and Payment Options webpages on the top menu of our website. Dove Physical Therapy, LLC will not schedule any new patients for initial Physical Therapy evaluations at 4:00 p.m. or after. If you or a loved one would like to receive physical therapy with us and schedule an appointment, please contact us at
