Dove Physical Therapy, LLC welcomes all new patients to the road of recovery. The first step towards recovery is having a referral or a prescription for Physical Therapy from a medical professional. Referring medical professionals consist of Medical Doctors, Doctors of Osteopathic Medicine, Obstetricians and Gynecologists, Neurologists, Physician Assistants, Nurse Practitioners, Dentists, Podiatrists, and Chiropractors. Patients may bring their Physical Therapy referral or prescription form to the clinic or request to have it faxed to the clinic at (334) 676-1652. Most importantly, although a physical therapy evaluation can be performed during the initial appointment, all patients must have a Physical Therapy referral or prescription form before treatment can began. Returning Patients

Dove Physical Therapy, LLC appreciates all returning patients who champions the obstacle of reaching their physical therapy goal. Continuing under the care of a physical therapist can make the journey a little comforting and much shorter to getting your lifestyle back or improving your daily living. If you were a previous patient and restarting treatment again, the process is the same as being a new patient. Patient Forms

Dove Physical Therapy, LLC would like to make its patient registration process as seamless as possible for our new and returning patients. Please complete and return all patient forms prior to the initial evaluation appointment. In order to access the patient forms, click on the Patient Forms button to access the Patient Forms Page. Then, scroll down and click on the Patient Intake Forms button to retrieve all the forms that need to be completed prior to your first appointment. If you prefer, you can request for the Patient Intake forms to be emailed to you or simply fill out the forms at your appointment.
